About This Formulation

Lipotropic injections combine Vitamin B12 (cyanocobalamin) with a blend of Methionine, Inositol, and Choline (MIC). These amino acids and nutrients are believed to support fat metabolism, liver function, and energy levels. This is a compounded formulation dispensed by a licensed pharmacy.

Contraindications

• Known hypersensitivity or allergy to cyanocobalamin (Vitamin B12), cobalt, or any component of the formulation
• Leber's hereditary optic neuropathy (Leber's disease): Patients with this condition should not receive cyanocobalamin, as it has been associated with rapid and severe optic atrophy
• Sulfite allergy: Some formulations contain sodium metabisulfite, which may cause serious allergic reactions including anaphylaxis or severe asthma attacks in susceptible individuals

Warnings and Precautions

• Anaphylaxis: Rarely, Vitamin B12 injections can cause anaphylactic shock, which is a life-threatening allergic reaction. Be prepared to seek emergency medical attention.
• Hypokalemia: In patients with severe megaloblastic anemia, aggressive treatment with B12 can cause low potassium (hypokalemia) and potentially serious cardiac complications. Serum potassium should be monitored during initial treatment.
• Benzyl alcohol: Some formulations may contain benzyl alcohol as a preservative, which has been associated with toxicity in neonates and is not recommended for use in premature infants or neonates.
• Polycythemia vera: B12 supplementation may mask the diagnosis of polycythemia vera.
• Folic acid deficiency: B12 supplementation may mask folic acid deficiency. Ensure adequate folate levels.
• Renal function: Patients with kidney disease should be monitored, as excretion of these compounds may be affected.

• Feeling of swelling of the entire body
• Mild itching or rash

Serious Side Effects

Seek medical attention immediately if you experience any of the following:

• Signs of anaphylaxis: difficulty breathing, hives, swelling of the face, lips, tongue, or throat, rapid heartbeat, dizziness, or loss of consciousness
• Chest pain or tightness
• Severe or persistent muscle weakness or cramps (may indicate low potassium)
• Irregular heartbeat
• Signs of a severe skin reaction: blistering, peeling, or a widespread rash
• Signs of pulmonary edema: difficulty breathing, wheezing, or coughing up pink or frothy mucus

Drug Interactions

• Metformin: Long-term metformin use may reduce Vitamin B12 absorption. Your provider may adjust your B12 supplementation accordingly.
• Proton pump inhibitors (PPIs) and H2 receptor antagonists: May reduce absorption of Vitamin B12 from the diet.
• Colchicine: May reduce B12 absorption.
• Aminosalicylic acid: May reduce absorption of Vitamin B12.
• Chloramphenicol: May interfere with the hematopoietic response to Vitamin B12.

Use in Specific Populations

• Pregnancy: Vitamin B12 is generally considered safe during pregnancy at recommended doses. However, the safety of the full MIC lipotropic formulation during pregnancy has not been established. Consult your healthcare provider.
• Lactation: Vitamin B12 is excreted in human breast milk. The effects of methionine, inositol, and choline supplementation on nursing infants are not fully established.
• Pediatric use: Safety and effectiveness in pediatric patients have not been established for lipotropic formulations.
• Geriatric use: Older adults are more likely to have decreased B12 absorption due to age-related changes. No specific dose adjustments are established.
• Renal impairment: Use with caution. Consult your healthcare provider.
