Important Safety Information — Minoxidil (Topical)

Indications

Topical minoxidil is indicated for the treatment of androgenetic alopecia (male pattern hair loss). It is applied directly to the scalp and works by stimulating hair follicles to prolong the growth phase and increase hair density. Results typically require at least 4 months of consistent use.

Contraindications

• Known allergy or hypersensitivity to minoxidil or any component of the formulation
• Do not apply to red, swollen, infected, irritated, or painful areas of the scalp
• Do not use if the cause of hair loss is unknown

Warnings and Precautions

• Systemic absorption: Skin conditions (irritation, sunburn, psoriasis) or applying to large areas may cause increased absorption of minoxidil into the body, potentially leading to systemic side effects. Do not use more than directed.
• Cardiovascular effects: Although topical application results in minimal systemic absorption in healthy individuals, patients with heart disease, heart failure, or hypertension should consult their healthcare provider before use, as oral minoxidil is a potent vasodilator.
• Initial hair shedding: Hair loss may temporarily increase during the first 2-4 weeks of treatment. This is a normal response and indicates that minoxidil is working by replacing resting hairs with new growth-phase hairs.
• Sun sensitivity: Minoxidil may make skin more sensitive to sunlight. Use sun protection on treated areas.
• Contact with eyes: Avoid contact with eyes, nose, or mouth. If accidental contact occurs, rinse thoroughly with water.
• Unwanted facial hair: Avoid allowing the solution to drip onto the face, as it can promote unwanted facial hair growth. Wash hands thoroughly after application.
• Scalp irritation: Formulations containing propylene glycol or alcohol may cause scalp dryness, itching, or irritation in some users.

Common Side Effects

• Scalp itching, dryness, scaling, or flaking
• Scalp irritation or burning
• Temporary increase in hair shedding (first 2-4 weeks)
• Unwanted hair growth on the face or body (if solution contacts those areas)
• Headache

Serious Side Effects

Stop use and seek medical attention immediately if you experience any of the following:

• Chest pain or rapid heartbeat
• Dizziness, lightheadedness, or fainting
• Sudden or unexplained weight gain
• Swelling of the hands, feet, or ankles
• Difficulty breathing, especially when lying down
• Severe scalp irritation or allergic reaction (rash, hives, swelling of face, lips, tongue, or throat, difficulty breathing)

Drug Interactions

• Guanethidine: Topical minoxidil may potentiate orthostatic hypotension caused by guanethidine. If concurrent use is necessary, patients should be monitored.
• Topical corticosteroids, retinoids, or other topical medications: Concomitant use on the scalp may increase absorption of minoxidil. Consult your provider about timing of application if using multiple topical products.
• Other vasodilators or blood pressure medications: Although systemic absorption from topical application is minimal, inform your provider of all medications, particularly antihypertensives.

Use in Specific Populations

• Pregnancy: Topical minoxidil should not be used during pregnancy. Adequate and well-controlled studies in pregnant women have not been conducted. Systemic minoxidil has been shown to reduce conception rates and cause fetal harm in animal studies.
• Lactation: Topical minoxidil is not recommended during breastfeeding, as systemic minoxidil is excreted in human milk.
• Pediatric use: Safety and effectiveness in patients under 18 years of age have not been established.
• Heart, kidney, or liver disease: Consult your healthcare provider before use.
• Hypertension: Patients with hypertension, especially those on blood pressure medications, should consult their provider before use.
